Completed paper health questionnaires:
If you do not use the online booking system then you will be asked to complete a paper health questionnaire, before the start of you first yoga session or therapy. Just like the online form this will cover name, address, contact details, and general health information. Your name and email will be added to the online booking system so that I can maintain my online register and manage my accounts. This will also allow you to receive reminder emails regarding any courses, workshops or therapies that you are booked onto.
Any other mailings regarding other courses or will only be sent to you if you tick the check box to say that you wish to join Yoga Therapy Stockport’s mailing list then your email address.
Specific to yoga therapy
For yoga therapy additional information is recorded, such as more detailed health and lifestyle information. Sessional notes are also recorded and kept by me as a requirement of my registration with the Complimentary and Natural Healthcare Council. These records are kept in a paper format only.
While yoga therapy most commonly refers to 1-2-1 session it can also include small group therapy courses such as yoga for anxiety and depression, or yoga for back care.
All information whether online or on paper is treated as confidential and is not shared with any third party. All paper records are stored securely by Yoga Therapy Stockport.
If you wish to alter any of your data, review how it is used or wish to request its return to you, please contact Susan Rodway-Hall at email@example.com or 07813 077074.
How long is your data stored?
It is a requirement of my insurance company that any personal data collected either online or on paper forms is kept for 7 years. This applies for all general yoga, Thai yoga massage and Indian head massage. For yoga therapy the Council Complimentary and Natural Healthcare Council stipulate that all records are kept for 8 years after receiving any yoga therapy.
